To prepare for your appointment, please print and fill-out Intake Forms to bring to your appointment. For your information, Privacy Practices are also available here. To provide you with accurate assessment and to avoid any drug-herb-supplement interactions, please bring in the list of your medications, including over-the-counter medications and supplements, with dosages, as well as any recent diagnostic work-up, such as laboratory work and/or diagnostic imaging.
Dr. Naydis is a preferred provider (in-network) with a number of health insurance companies, including AETNA, Cigna, First Choice, Healthways, Premera, and Regence. For other insurances, you may be eligible for out-of-network benefits for your visit. Please, check with your insurance carrier for naturopathic or acupuncture coverage.
